Renal Dose Adjustment
Look up dose adjustments for 50 drugs across 5 CrCl tiers, including which drugs are contraindicated.
This is a lookup tool, not a calculator with a formula — it holds pre-defined dose adjustment guidance for 50 drugs across 5 kidney-function tiers. See How Calculator Tools Work for the shared layout.
How to use it
- Search or select a drug from the list of 50 (by generic or brand name).
- Enter the patient's CrCl — or arrive here directly from a Creatinine Clearance result, which carries the value across automatically.
The tool maps the CrCl to one of 5 tiers and shows that drug's guidance for that tier:
| CrCl (mL/min) | Tier |
|---|---|
| ≥ 60 | Normal |
| 45–59 | Mild |
| 30–44 | Moderate |
| 15–29 | Severe |
| < 15 | ESRD |
What the result shows
For the selected drug and tier: the recommended dose (which may be "no adjustment needed," a reduced dose, an extended interval, or contraindicated — flagged explicitly when a drug shouldn't be used at that renal function level at all), a short note explaining the adjustment, and a monitoring recommendation specific to that drug.
If a drug is marked contraindicated at the patient's tier, that's not a suggestion to reduce the dose further — it means an alternative drug should be considered instead.
