Start a direct, group, or patient-query conversation.

A direct conversation must have exactly 2 participants — trying to start one with more (or fewer) is rejected with "Direct conversations must have exactly 2 participants." Use a group conversation for anything larger.
Starting a clinical consultation conversation is restricted to pharmacists, owners, pharmacy outlet managers, and pharmacy team roles — "Only pharmacists, owners, pharmacy outlet managers, and pharmacy team can create clinical consultations."
