Approve, suspend, or reactivate a patient's portal account.

Lists every patient portal account, filterable, with individual Approve, Suspend, and Reactivate actions.
Two actions here aren't functional yet — both return "Not yet implemented": bulk-approve (approve accounts one at a time instead) and exporting the patient list. Individual approve/suspend/reactivate all work normally. Where bulk operations or exports do work elsewhere in the portal, they're capped at 10/hour and 5/day respectively.
