Create an Intervention

Log a clinical intervention — the issue, its priority, and up to 10 strategies to address it.

Create an intervention from the Interventions Dashboard or directly from a patient's record.

Required fields

  • Patient
  • Category — Drug Therapy Problem, Adverse Drug Reaction, Medication Non-adherence, Drug Interaction, Dosing Issue, Contraindication, or Other
  • Issue description — 10 to 1,000 characters
  • Priority — Low, Medium, High, or Critical

Adding strategies

Optionally add up to 10 strategies for how you'll address the issue. Each strategy has:

  • Type — Medication Review, Dose Adjustment, Alternative Therapy, Discontinuation, Additional Monitoring, Patient Counseling, Physician Consultation, or Custom
  • Description (1–500 characters) and rationale (1–500 characters)
  • Priority — Primary or Secondary

Two strategy rules to know: you can't add two strategies of the same type to one intervention, and a Custom strategy needs both its description and rationale to be at least 20 characters each — the general 1-character minimum doesn't apply to Custom.

Duplicate detection

If a very similar intervention already exists for this patient, creating a new one is blocked with a duplicate-entry error — check Interventions by Patient first if you're not sure whether one already exists.
