Common validation errors for interventions, DTPs, and ADR reports.

"Duplicate entry detected" when creating an intervention

A very similar intervention already exists for this patient. Check Interventions by Patient before creating a new one, or open the existing one instead.

"Duplicate strategy types are not allowed"

You can't add two strategies of the same type to one intervention — for example, two "Dose Adjustment" strategies. Combine them into one, or use a different type for the second.

Why was my Custom strategy rejected?

Custom strategies need both description and rationale to be at least 20 characters each — the usual 1-character minimum for other strategy types doesn't apply to Custom.

What's the difference between symptom severity and overall ADR severity?

An ADR report has two separate severity scales: each individual symptom is Mild/Moderate/Severe, while the report's overall severity adds two more levels above that — Life-threatening and Fatal. Rate them independently; a report with only mild individual symptoms can still be marked overall-severe if the combination warrants it.

"At least one drug is required" / "At least one symptom is required"

Both are hard requirements on an ADR report — an ADR with an empty drug or symptom list can't be saved, even as a draft.

Do DTPs and MTR problems use the same categories?

Yes — DTPs and MTR's problem-identification step share the exact same type/category/severity taxonomy, so what you learn recording one applies directly to the other.
