Adding a DTP

Only type is required — everything else is optional:

• Type — Unnecessary, Wrong Drug, Dose Too Low, Dose Too High, Adverse Reaction, Inappropriate Adherence, Needs Additional Therapy, Interaction, Duplication, Contraindication, or Monitoring (the same taxonomy used for MTR problems)
• Category — Indication, Effectiveness, Safety, or Adherence
• Severity — Critical, Major, Moderate, or Minor
• Description and clinical significance — up to 1,000 characters each
• Evidence level — Definite, Probable, Possible, or Unlikely
• Affected medications, related conditions, risk factors
• Status — defaults to Identified; other values are Addressed, Monitoring, Resolved, Not Applicable, or Unresolved
