Submitting an ADR Report
Report an adverse drug reaction — required fields and the two different severity scales involved.

Required fields
- Patient
- At least one drug — name required; flag whether it's the suspected cause
- Reaction description
- At least one symptom — name and severity (Mild, Moderate, or Severe) each
- Overall severity — Mild, Moderate, Severe, Life-threatening, or Fatal
- Onset date
- Reporter type — Pharmacist, Physician, Nurse, Patient, or Other
There are two different severity scales in this form: each individual symptom is rated Mild/Moderate/Severe, while the report's overall severity has two additional levels above that (Life-threatening, Fatal). Don't confuse the two when filling in the form.
Optional fields
- Outcome — Recovering, Recovered, Not Recovered, Recovered with Sequelae, Fatal, or Unknown
- Patient demographics — age, gender, weight (all optional, in case they're not already on file)
