New Diagnostic Case (4-Step Intake)
Enter a patient's symptoms, vitals, and history to start an AI diagnostic analysis.

Starting a diagnostic case is a 4-step form. Despite the name "case" in the UI, this creates a single diagnostic request — there's no separate multi-visit case record behind it.
Step 1 — Patient Selection
Search for and select the patient this analysis is for.
Step 2 — Symptom Assessment
- Subjective symptoms (required) — at least 1, up to 20, each up to 500 characters
- Objective findings — up to 20, each up to 500 characters
- Duration — free text, up to 100 characters
- Severity — mild, moderate, or severe
- Onset — acute, subacute, or chronic
Step 3 — Vital Signs & History
- Vitals (all optional): blood pressure (
systolic/diastolicformat), heart rate, temperature, blood glucose, respiratory rate, oxygen saturation, weight, height — each with a validated clinical range - Current medications — up to 50, each with name (required), dosage, frequency, route, start date, indication
- Allergies — up to 20, each up to 100 characters
- Medical history — up to 30 items, each up to 500 characters
- Family history — up to 20 items
- Social history — smoking, alcohol use, and exercise level
- Lab results — attach up to 20 existing lab results from the patient's record
- Priority — routine, urgent, or stat
Step 4 — Review & Consent
A summary of everything entered, plus a required consent confirmation — the request cannot be submitted without it.
Patient consent is required. Submitting without checking the consent box is rejected with "Patient consent is required for AI diagnostic processing."
After submitting
You're taken to Case Results & AI Analysis, which polls for the AI's output.
