Identifying a problem
For each drug therapy problem (DTP) you identify:
- Category — Indication, Effectiveness, Safety, or Adherence
- Type — one of: Unnecessary, Wrong Drug, Dose Too Low, Dose Too High, Adverse Reaction, Inappropriate Adherence, Needs Additional Therapy, Interaction, Duplication, Contraindication, or Monitoring
- Severity — Critical, Major, Moderate, or Minor
- Description — 10 to 1,000 characters
- Clinical significance — required, explaining why it matters for this patient
