ClinicalMedication Therapy Review (MTR)

Step 2: Therapy Assessment

Check for interactions and duplicate therapies, then identify drug therapy problems.

Step 2 is where you actually assess the medication list you built in Step 1.

Automatic checks

Run interaction and duplicate therapy checks against the full medication list — both surface directly in this step so you can review them before identifying problems.

Identifying a problem

For each drug therapy problem (DTP) you identify:

  • Category — Indication, Effectiveness, Safety, or Adherence
  • Type — one of: Unnecessary, Wrong Drug, Dose Too Low, Dose Too High, Adverse Reaction, Inappropriate Adherence, Needs Additional Therapy, Interaction, Duplication, Contraindication, or Monitoring
  • Severity — Critical, Major, Moderate, or Minor
  • Description — 10 to 1,000 characters
  • Clinical significance — required, explaining why it matters for this patient
