FAQ & Troubleshooting: MTR
Common validation errors and access questions for Medication Therapy Review.
"Patient already has an active MTR session"
Only one active session per patient at a time. Find and finish (or close) the existing one from Patient MTR Sessions List before starting another.
"Strength must include both value and unit"
In Step 1, a medication's strength needs a number and a unit (for example, "500" and "mg") — one without the other is rejected.
"Therapy plan must be linked to at least one drug therapy problem" / "At least one recommendation is required"
Step 3 needs both a linked problem and a recommendation before you can move on — having only one of the two isn't enough.
"Prescriber communications must use written, phone, email, or fax methods"
Only shows up in Step 4 when you're logging a communication specifically to a prescriber — pick one of the four listed methods.
I have access to MTR on one patient's profile, but the button is missing entirely
That's a plan issue, not a permission issue — see Understanding MTR Access Requirements for the difference between the two gates and how to tell which one is blocking you.
Why can't I add another problem or recommendation partway through?
You can — there's no cap on how many problems, recommendations, or interventions you record; the required minimums (at least one problem, at least one recommendation) are a floor, not a ceiling.
