- Type — Recommendation, Counseling, Monitoring, Communication, or Education
- Category — Medication Change, Adherence Support, Monitoring Plan, or Patient Education
- Description — up to 1,000 characters
- Rationale — required, up to 1,000 characters, explaining why
If you're recording a communication to a prescriber, you must specify how it was sent — Written, Phone, Email, or Fax. This is only required when the target audience is specifically the prescriber.
