Step 1: Medication History
Record everything the patient is currently taking — the foundation the rest of the review builds on.

The first step of an MTR session is building a complete list of the patient's current medications.
Required fields per medication
Every medication entry needs all of the following — an entry missing any of these is rejected:
- Drug name
- Strength — both a numeric value and a unit (for example, "500" + "mg"); a strength with only one of the two is rejected
- Dosage form (tablet, syrup, injection, and so on)
- Instructions
- Category
- Start date
- Indication — why the patient is taking it
Why this step matters
Everything downstream — interaction checks, duplicate-therapy checks, and the problems you identify in Step 2 — is checked against this list. An incomplete or inaccurate medication history here weakens the whole review.
