Care Plans Tab

Set structured goals and objectives for a patient's ongoing care, with a review date and status.

A care plan records what you're working toward with a patient and the concrete steps to get there.

Creating a care plan

  • Goals — 1 to 10 of them, each 5–200 characters (for example, "Achieve HbA1c below 7% within 6 months")
  • Objectives — 1 to 15 of them, each 5–300 characters (the concrete actions that support the goals)
  • Follow-up date — when the plan should next be reviewed
  • Plan quality — flagged as Adequate or Needs Review
  • Status — Resolved or Unresolved

A care plan can optionally be linked to a specific visit.

Reviewing a care plan

Update the status to Resolved once the goals have been met, or keep it Unresolved and adjust the objectives as care continues — there's no separate "edit history," each save reflects the plan's current state.
