Adding a Custom Condition
Define a chronic condition specific to your workspace, with its own monitoring fields and suggested interventions.
If your pharmacy manages a chronic condition outside the 9 built-in ones, add it as a custom condition from the Condition Registry page.
Fields
- Name (required)
- Description
- Category — defaults to "other" if not set
- Color — for the condition's badge/chart color, defaults to a neutral grey if not chosen
- Monitoring fields — the values you'll record for this condition at each encounter (numeric, select, boolean, or text)
- Interventions — a list of suggested actions pharmacists see when this condition comes up as uncontrolled
What happens after you save
Your custom condition gets a generated ID from its name (e.g. "Post-Stroke Care" becomes custom_post_stroke_care) and appears in the Condition Registry alongside the built-in 9 — available to select in Encounter Wizard Step 1 exactly the same way.
Condition names must be unique within your workspace. Trying to save a second condition with a name that already exists — including a name that only differs by capitalization or spacing — is rejected with "A condition with this name already exists in your workspace."
Unlike the built-in 9, custom conditions don't have automatic "uncontrolled" logic or auto-detection from medications — those are hand-coded per built-in condition. A custom condition's status is set manually.
Editing or removing a custom condition
Custom conditions can be edited or removed from the same registry screen. Removing one doesn't delete history already recorded against it — it stops appearing as a selectable option for new encounters.
