Encounter Wizard Step 5: Interventions
Record what action you took, including referring the patient to a physician.
Step 5 records the action taken in response to what you found — often the problem you just logged, but not required to reference one.
Fields
- Type (required) — Counsel Patient, Adjust Dose, Switch Drug, Add Medication, Refer to Physician, Order Lab Test, Schedule Follow-up, Provide Education, or Lifestyle Modification
- Description (required, up to 1000 characters)
- Related condition
- Related medications — link one or more medications this intervention concerns
Each condition suggests its own interventions based on your entries — a hypertension visit flagged as Stage 3 will suggest referral; an uncontrolled diabetes visit will suggest reviewing the current regimen or referring if HbA1c is over 8%. These are suggestions to speed up documentation, not requirements — you can record any intervention type regardless of what's suggested.
Referring to a physician
Refer to Physician is one of the 9 intervention types, not a separate flow — pick it, describe why, and it's recorded on the encounter like any other intervention.
Updating an intervention's status later
After the encounter, an intervention can be marked accepted, declined, or completed from Encounter Detail — useful when the action (like a physician referral) takes time to actually happen.
