Fields
- Problem tag (required) — one of: Uncontrolled Condition, Non-Adherence, Adverse Drug Reaction, Drug Interaction, Duplicate Therapy, Suboptimal Dosing, Missing Monitoring, Worsening Trends
- Description (required, up to 1000 characters)
- Severity — low, medium, high, or critical (optional)
- Related condition — link the problem to a specific condition from this encounter, if relevant
If a problem was surfaced automatically rather than typed by you — for example, an uncontrolled-condition flag the system already raised — it's marked as auto-detected so you can tell it apart from problems you identified yourself during the visit.
