Encounter Wizard Step 4: Problem Identification

Tag and describe any drug-therapy problems found during this encounter.

Starter+

Step 4 is where you log any problem identified during the visit — separate from the intervention you'll record for it in the next step.

Fields

  • Problem tag (required) — one of: Uncontrolled Condition, Non-Adherence, Adverse Drug Reaction, Drug Interaction, Duplicate Therapy, Suboptimal Dosing, Missing Monitoring, Worsening Trends
  • Description (required, up to 1000 characters)
  • Severity — low, medium, high, or critical (optional)
  • Related condition — link the problem to a specific condition from this encounter, if relevant

If a problem was surfaced automatically rather than typed by you — for example, an uncontrolled-condition flag the system already raised — it's marked as auto-detected so you can tell it apart from problems you identified yourself during the visit.

Step 5: Interventions — where you record what you did about the problem(s) logged here.
