What's shown
- Fields tracked — every value recorded for this condition at each encounter, with its type, unit, and valid range.
- "Uncontrolled" logic — the exact thresholds that flag this condition as uncontrolled. A few real examples:
- Hypertension: systolic ≥140 or diastolic ≥90 (staged further at ≥160/100 as Stage 2 and ≥180/110 as a hypertensive crisis).
- Type 2 Diabetes: fasting glucose >126 mg/dL, random glucose >200 mg/dL, or HbA1c >7%.
- Asthma: daily symptoms, or any night symptoms.
- Hyperlipidemia: total cholesterol >5 mmol/L or LDL >3 mmol/L.

Why this matters before an encounter
Knowing a condition's thresholds ahead of time tells you what values actually change its status — for example, Type 2 Diabetes only flips to uncontrolled on fasting glucose, random glucose, or HbA1c specifically; recording only a symptom note without one of those three values won't trigger the flag no matter how it reads clinically.
