The Chronic Disease Dashboard is the entry point for managing patients with ongoing conditions — hypertension, diabetes, asthma, and 6 others tracked in the system. It shows what needs attention across your whole workspace, not one patient at a time.
What's on it
- Uncontrolled conditions — every condition currently flagged
uncontrolled, worst-first (most alerts on top). See Understanding Auto-Detect Suggestions for how a condition gets flagged. - Recent encounters — the latest chronic disease visits logged across the workspace.
- Follow-up reminders — patients whose scheduled follow-up date has passed with no subsequent encounter recorded — these are genuinely missed, not just upcoming.
- Condition distribution — a breakdown of how many patients are tracked per condition.
Where to go next
- Start a new visit from Encounter List or directly from a patient's own Chronic Disease tab.
- Browse the condition catalogue and add custom conditions from Condition Registry.
