Encounter Detail
The full read-only record of a chronic disease encounter — conditions, vitals, problems, interventions, and follow-up.
Encounter Detail is the permanent record of one visit — everything captured across the 6-step wizard, laid out for review rather than editing.
What's shown
- Conditions covered in this encounter
- Vitals and condition-specific values recorded
- Medications reviewed, added, or removed
- Problems identified, with tag and severity
- Interventions recorded, with their current status
- Follow-up date, type, and notes, if one was set
Updating an intervention after the visit
Interventions can still be updated from here after the encounter is completed — mark one accepted, declined, or completed as the real-world outcome becomes known (for example, marking a physician referral as completed once the patient actually sees the doctor), with an optional note up to 500 characters.
What can't be changed here
Vitals, problems, and conditions are locked once the encounter is completed — Encounter Detail is a record, not an editable form. If something needs correcting, log it in a new encounter rather than editing this one.
