Common questions about condition flagging, encounters, and auto-detection.
Why did this condition suddenly get flagged as uncontrolled?
Each condition has its own trigger — see Condition Config Detail for the exact thresholds. It's checked automatically after every completed encounter using the most recent recorded values, so a single high reading on the right field (e.g. systolic ≥140 for hypertension) is enough to flip the status, even if everything else about the visit looked fine.
"A condition with this name already exists in your workspace"
You're trying to add a custom condition with a name that's already in use in your workspace — including one that only differs by capitalization. Pick a distinct name.
"Systolic BP must be greater than diastolic BP"
You've entered the two values swapped during Vitals Entry — double check which number is which.
I completed an encounter but need to change something in it
You can't edit a completed encounter directly — Encounter Detail is a read-only record except for updating an intervention's status. If something was wrong, log a new encounter with the correct information rather than trying to reopen the old one.
"Follow-up date must be in the future"
Step 6 only accepts a future date — you can't schedule a follow-up for today or a past date.
Why doesn't the auto-detect suggestion match what I'd expect?
Auto-detect only looks at active medications, not diagnoses or notes — see Understanding Auto-Detect Suggestions. If a patient's medication for a condition was recently discontinued, or their diagnosis isn't reflected in a matching drug yet, the suggestion won't appear.
A follow-up date passed and nothing happened — is that a bug?
No — a missed follow-up isn't chased automatically. It shows up as a reminder on the Chronic Disease Dashboard, but reaching out to the patient is still a manual step.
The trend chart says "stable" but the numbers look worse to me
Trend direction needs at least 2 data points and treats small movement as noise rather than a real trend — see Trend Visualization. With very few encounters recorded, or values moving only slightly, "stable" can undersell what's actually happening. Read the raw values alongside the trend label, especially early in a patient's tracking history.
