Trend Visualization

See how a patient's condition values have changed across encounters over time.

Trend Visualization charts a patient's recorded values for a condition across every encounter — turning a series of individual visits into a picture of whether they're improving, stable, or getting worse.

What it shows

Pick a patient and a condition to see the relevant tracked values plotted over time — for hypertension, that's systolic and diastolic BP across visits; for diabetes, fasting glucose and HbA1c; and so on per condition, matching the fields defined in Condition Config Detail.

The direction shown (up, down, or stable) is calculated with a simple linear regression across the patient's last 5 recorded values for that metric — it needs at least 2 data points to say anything at all, and treats small movement as "stable" rather than reading noise as a trend.

Trend direction is descriptive, not a diagnosis — a metric "trending up" toward an already-normal range is a good sign, while one trending up from an already-high baseline is a warning. Read the direction alongside the actual values, not on its own.

From a patient's Chronic Disease tab, open Trend Visualization for any condition they're being tracked for.
