A patient's chronic conditions, recent encounters, and auto-detect suggestions, all from their own profile.
Starter+
The Chronic Disease tab on a patient's profile is the fastest way into this module for a specific patient — everything here is scoped to just them, rather than the workspace-wide views on the Dashboard and Encounter List.
What's on it
- Active conditions — every condition currently tracked for this patient, with status (active, managed, controlled, uncontrolled, or resolved).
- Auto-detect suggestions — conditions the system thinks this patient might have based on their current medications, not yet being tracked. See Understanding Auto-Detect Suggestions.
- Recent encounters — this patient's chronic disease visit history.
- Start Encounter — begins the 6-step wizard with this patient pre-selected.
Flagging or unflagging a condition manually
A condition can be flagged or unflagged as uncontrolled directly from this tab, separately from the automatic flagging that happens after each encounter — useful if something outside a formal encounter (a phone call, a lab result reviewed elsewhere) changes your assessment.
