Encounter Wizard Step 2: Vitals Entry

Record vitals and condition-specific values — the exact fields shown depend on which condition you selected.

Step 2 shows a form built from the condition(s) you picked in Step 1 — every condition defines its own fields, so this screen looks different for a hypertension visit than a diabetes visit.

General vitals (any condition)

FieldValid range
Systolic BP70–300 mmHg
Diastolic BP40–200 mmHg (and must be lower than systolic)
Heart rate30–220 bpm
Weight1–500 kg
Height30–250 cm
Temperature30–45°C
Respiratory rate5–60
Oxygen saturation50–100%
Fasting glucose20–600 mg/dL
Random glucose20–800 mg/dL

Condition-specific fields

Each condition adds its own values on top of the general vitals above — for example, Hypertension asks for adherence (good/fair/poor) and symptoms; Type 2 Diabetes asks for HbA1c and diet compliance; Asthma asks for symptom frequency, night symptoms, and peak flow. See Condition Config Detail for any specific condition's full field list.

Systolic BP must be greater than diastolic BP — entering them the wrong way round is rejected before you can continue.
